9/23/2023 EnchantaVaughn Approved Behavior: During the first part of his therapy, client was seen on task and exhibiting a good attitude. Client expressed positive thoughts to his counselor and seemed to be focused and alert during the role play activity. Client continues to be disrespectful. Behavior: Client was observed making good choices and avoiding negative behaviors. Client appeared to be in an ecstatic mood, as evidenced by his enthusiasm and positive expressions. Client was seen displaying inappropriate social interactions during the session. Intervention: QMHP implemented individualized behavior management techniques with client. QMHP expressed a cheerful attitude toward client. QMHP processed with client about psychoeducational activity while assisting with directions. QMHP explained the importance of the activity. QMHP provided feedback to client in regard to questions asked. QMHP explored clientâs responses and provided appropriate feedback. QMHP supported client making good decisions and having positive behaviors at home. Intervention: QMHP supported client with his objectives and coping with stressors without needing additional redirection or prompts. QMHP complimented client for utilizing acquired skills, paying attention, and focusing on the importance of the assignment. QMHP supported client displaying positive social interaction with authority. QMHP modeled proper positive behavioral expectations for the client to encourage additional layers of support. QMHP commended client on his efforts to maintain his ISP objectives throughout the day. Response: Client displayed respectful, non-aggressive behaviors in the home. All objectives were being met at this time, despite stressful environment. Client avoided distractions in social setting, while utilizing anger management skills. Client seem to be heavily influenced by others and pressured into doing things he shouldnât do. Client displays leadership skills by helping others and being good. Client acknowledged interventions and verbalized clear understanding of each intervention. Response: Client engaged in reflective thinking and expressed his feelings to QMHP. Client communicated the do's and don'ts of home/school rules so that he shows more commitment to his choices. Client was in a pleasant mood after feedback was given. Client was seen having a good time. Client was able to identify the role he plays in each situation and continues to comply with authority rules and regulations. Client remembered to apply himself and utilize his strengths to focus on meeting the goals listed on his ISP.
